Abstract:View Full Presentation InformationBreast abscesses that occur during breastfeeding/chestfeeding are a relatively rare complication. However, they can significantly affect and permanently impair the dyad. In this lecture, we will discuss the causes, diagnosis and treatment of lactational abscesses, and ways to prevent them. In addition to the more common causes of abscesses during lactation, we will also discuss rarer forms such as idiopathic granulomatous mastitis or tuberculosis of the breast and their treatment.
The necessary examination methods and therapeutic options must also be adapted to the specific characteristics of breastfeeding/chestfeeding. -

This presentation examines the wide range of potential contributors to lactation difficulties, with particular attention to hormonal influences. Where do we look when poor breast stimulation has been ruled out or corrected and milk supply is still not increasing? Learn more about the multiple hormonal issues that can impact milk production. We will also explore the different types of breast hypoplasia, beyond just abnormal development during puberty. Participants will expand their knowledge of the varied causes of low milk production allowing for more nuanced differential diagnosis and appropriate clinical management or referral.

Dr. Amy Fung is a general practitioner and has become an International Board Certified Lactation Consultant since 2009. She has germinated interest in lactation management during the period she worked in government Maternal and Child Health Centre. The passion has been deepened after fully breastfed her three children. Her particular interests are feeding and sleeping issues of infant and young child; breast pain and recurrent plugging/mastitis of lactating women.
She has passion for breastfeeding education. She was the course director of the “train-the-trainer” Breastfeeding Course organized by World Health Organization UNICEF in 2014. It is presented with a spectrum of clinical presentation, from one-off acute episodes to less severe but recurrent attacks. Plugging with a painful focal induration occurs in both acute and subacute mastitis. Systemic symptoms like fever, chills and malaise are less common in subacute mastitis. Other symptoms like chronic breast pain, recurrent nipple bleb, congealed milk or bloody milk are reported in subacute mastitis.
The aetiology, pathophysiology, classification and treatment of mastitis are debatable. Milk stasis in a milk duct has been postulated to be a potential provoking factor of mastitis. Does the breast lump represent a single milk duct being blocked by a macroscopic milk “plug” similar to a tube of toothpaste? Is deep massage an effective way to solve a “blocked duct”?
Studying the anatomy of the lactating breast lets us understand the pathophysiology of the mastitis spectrum. The effectiveness of various treatment modalities are evaluated, such as deep breast massage and unroofing nipple blebs. A comprehensive approach to managing subacute mastitis with recurrent plugging could protect against premature termination of lactation and hence safeguard infant and maternal health. -

Andrea Herron, is one of the first and longest continuous certified pediatric nurse practitioners in the United States. After more than 40 years working with breastfeeding mothers and their babies and teaching parenting classes, she is among one of the most experienced consultants in the field of lactation. Regardless of the issue or concern, Andrea has guided thousands of mothers to meet their breastfeeding and early parenting goals through support groups, lactation consultations, and childrearing education. After receiving a Master's in nursing from UCLA, during which she concentrated her studies in infant development and feeding issues Andrea became an early pioneer in the back-to-breastfeeding movement, and educated health professionals as an instructor in the UCLA lactation educator course all over the United States. Her private lactation practice, Growing with Baby in San Luis Obispo, California, was used as the national model for private practices by Women Infant and Children (WIC), the federally funded health and nutrition program. This session provides an updated look at the pathophysiology, contributing factors and clinical management of hyperlactation. Topics will include the types, causes and pathophysiology of hyperlactation, the process of taking a detailed history and doing a thorough exam and care plan development. Learn more about the complications of hyperlactation, going beyond just mastitis and also looking at milk composition, research on the impact of pumping on milk supply and breast health.
Participants will also learn about when to refer and suggested medical workup and management of hyperlactation that doesn’t respond to supportive measures. -

Many conditions are painful, or itchy, and can be distressing, particularly for individuals who are breastfeeding. This talk will briefly explain skin structure and consideration of skin colour and examine the identification of common skin conditions with a focus on dermatitis and infections. The history of the problem will often indicate the diagnosis, which may be confirmed on examination. Management involves explanation, advice about good skin care, and safe use of medication.
